چکیده

Introduction It has previously been reported that patients with an implantable cardioverter-defibrillator (ICD) may have a slightly higher defibrillation threshold (DFT) in an upright position compared to a supine position. However, the significance of this and the potential impact on clinical practice remains unclear. We report a case of defibrillation failure caused by an increase in DFT in the upright position, leading to prolonged, near-fatal cardiac arrest in a patient who had previous ICD implantation including successful DFT testing in a supine position. ICD system revision with addition of an azygos vein coil successfully corrected this condition, as demonstrated by orthostatic DFT testing.

The potential mechanism of the detrimental effect of defibrillation prior to cardiopulmonary resuscitation in prolonged cardiac arrest model

Defibrillation is no longer universally recommended as initial intervention for the reversal of ventricular fibrillation (VF) after a prolonged and untreated cardiac arrest. We sought to examine this issue in an animal model where a prolonged untreated VF was induced. The aim of this study was to investigate the potential mechanism of the detrimental effect of defibrillation prior to cardiopulm...

BRIEF REVIEW Determinants of successful defibrillation

Ventricular defibrillation by electric counter shock is the definitive treatment for cardiac arrest due to ventricular fibrillation (VF). Ventricular defibrillation is the fundamental cornerstone to a successful outcome. As most deaths from VF occur out of hospital, often in patients without pre-existent recognised cardiac disease, the major public health issue remains the widespread applicatio...

Advanced cardiac life support: what's new, what's old?

Approximately 300,000 out-of-hospital cardiac arrests occur in the United States each year. The American Heart Association (AHA) supports implementation of the “chain of survival” to rescue people who experience cardiac arrest in the community. The “chain of survival” consists of early recognition and emergency activation (phone 9-1-1), CPR, defibrillation, and advanced care. Advanced Cardiac L...

The Role of Automated External Defibrillators in Athletics

CONTEXT Sudden cardiac arrest is the leading cause of death in young athletes. The purpose of this review is to summarize the role of automated external defibrillators and emergency planning for sudden cardiac arrest in the athletic setting. EVIDENCE ACQUISITION Relevant studies on automated external defibrillators, early defibrillation, and public-access defibrillation programs were reviewed...

Public access defibrillation: a review.

Sudden cardiac death remains a worldwide health problem. More than 1000 such deaths occur daily in the United States; most are secondary to ventricular fibrillation (VF). Unfortunately, only about 5% of victims survive. Sudden cardiac death is the most common “accidental” death in the US. A combined total of all airline, automobile, and drowning deaths would not equal the number of accidental o...
